Shirodhara is a steady stream of warm medicated liquid poured across the forehead for 30 to 45 minutes. Small clinical studies link it to lower anxiety and calmer stress markers, and in our clinic it works best for stress-driven anxiety with poor sleep, used alongside proper medical care rather than in place of it.

The person who books Shirodhara for anxiety at our Gachibowli branch often looks the same. Early thirties. Laptop bag still on one shoulder. Asleep by midnight, wide awake at 3 a.m., already rehearsing tomorrow’s meetings in their head.
They’ve usually tried the meditation apps. Some are on medication. Almost all of them ask the same thing before they lie down: will a stream of warm oil on my forehead actually do anything for anxiety?
We’ve been giving Shirodhara at Arooda Kerala Ayurveda for 17 years, across Gachibowli, Banjara Hills and Manikonda. Stress, anxiety and broken sleep now bring in more Shirodhara patients than any other complaint we see. This piece covers what happens on the table, what the published studies show and what they don’t, and the cases where we tell people to wait.
The short version. It helps many people calm down and sleep better. It does not cure an anxiety disorder. And it isn’t right for everyone.
What Is Shirodhara, and Why Is It Used for Anxiety?
The name joins two Sanskrit words, Shiras (head) and Dhara (flow). A thin, unbroken stream of warm liquid falls from a hanging vessel onto your forehead while the therapist swings it slowly from side to side. No pressure. No kneading. Just the stream.
Classical texts place it among the Murdha Taila therapies, the group of oil treatments aimed at the head, described in Ashtanga Hridaya (Sutrasthana, chapter 22). Kerala physicians refined the equipment and the rhythm over generations, which is why a Kerala clinic’s setup looks the way it does.
The Ayurvedic reading of anxiety
Ayurveda doesn’t have a single word that maps neatly onto the modern diagnosis. The closest classical term is Chittodvega, which Charaka Samhita lists among disorders of the mind. Physicians tie it to disturbed Vata, and in particular Prana Vata, the subtype said to govern the head, the senses and the mind.
Racing thoughts. Light, broken sleep. Cold hands. A dry mouth before a presentation. These are classic Vata signs, and they also describe most of our anxious patients pretty well.
The qualities that settle Vata are warmth, oiliness, steadiness and slowness. Shirodhara brings all of them to the head at once.
Why the stream lands on the forehead
The stream falls near the point between the eyebrows, a region Ayurveda treats as a vital point, Sthapani Marma. The modern explanation is more down to earth. The forehead is rich in nerve endings, and warm, rhythmic, predictable touch there appears to pull the nervous system out of alert mode.
Researchers at the University of Toyama measured this. After 30 minutes of Shirodhara, plasma noradrenaline dropped and foot skin temperature rose, which they read as reduced sympathetic, or “fight or flight,” activity (Uebaba et al., Journal of Alternative and Complementary Medicine, 2008). Not because of tradition. Because of physiology.
What Happens During a Shirodhara Session?
First-timers usually worry about the wrong things. Oil getting into their eyes. Having to stay awake. Driving home with an oily head. This is how a session at Arooda runs, start to finish.
Before the oil: the consultation
Every new patient sees the physician first. We ask about sleep, digestion, bowel habits, how the anxiety shows up in your day, what medication you take and if you’ve ever had a head or neck injury. We check the pulse and look at the tongue.
This is where the liquid gets chosen. It could be a medicated oil, medicated milk (Ksheeradhara), medicated buttermilk (Takradhara) or a herbal decoction. Your constitution, digestion, the season and the kind of anxiety you have all feed into that decision. Two people with the same complaint can walk out with different prescriptions, and that’s normal.
On the table
You lie on your back on a wooden Droni, the traditional Kerala treatment table. Soft cotton pads cover your eyes, and a band around the forehead makes the oil run back into your hair instead of your face. Some patients get a short head and shoulder Abhyangam first.
The vessel, called the Dhara Patra, hangs a few inches above you. Classical guidance puts the height at about four finger-widths (Angula). The therapist keeps the stream moving in a slow arc and keeps re-warming the oil so the temperature holds. In a study at the ICMR Advanced Centre in Mumbai, oil was kept at roughly 40°C with a thermostat (Dhuri, Bodhe and Vaidya, Journal of Ayurveda and Integrative Medicine, 2013).
The first five minutes feel a bit odd. Then something shifts. Most people describe a heavy kind of calm, awake but not really thinking. Some drift off. Both are fine. Volunteers in the Mumbai study said much the same, reporting relaxed alertness with occasional mild drowsiness.

Timings vary with your prescription. Plan on about 90 minutes at the clinic for a first visit.
After the stream stops
You’ll rest before sitting up. Getting up fast after 40 minutes of deep relaxation can make you lightheaded. We wipe the face and forehead, but the scalp oil usually stays on for a while. Our standard aftercare:
- Wash your hair with lukewarm water, never cold, after the wait time your therapist gives you.
- Cover your head on the way home and skip the car AC on full blast.
- Eat something light and warm. Leave heavy, fried food for another day.
- Don’t book a stressful call straight after. Many people take the last slot of their day for exactly this reason.
- Sleep early that night if you can.
Does Shirodhara for Anxiety Actually Work? What the Research Shows
The studies are small. Most have fewer than 30 people. Still, they point the same way, and a couple were designed with real care.

A note on oils. The studies above used specific oils because a study needs a fixed recipe. That doesn’t make any of them a prescription. At Arooda the physician decides the liquid after assessment, because the right choice changes from person to person.
What these studies can’t tell you
Samples are tiny, and several used healthy volunteers rather than people with a diagnosed anxiety disorder. Blinding is close to impossible too. You know when warm oil is on your head. The US National Center for Complementary and Integrative Health notes that well-designed clinical trials on Ayurvedic approaches are few. We agree with that, which is why we describe Shirodhara as support for anxiety and not a stand-in for diagnosis and treatment.
What we see in the treatment room
Sleep tends to move first. By the third or fourth session, patients often tell us they’re falling asleep faster, even when the daytime worry hasn’t budged. Jaws unclench. Tension headaches back off. The daytime anxiety usually follows a week or so later.
People whose anxiety is tied to a clear stressor, like appraisal season or a house move, usually respond better than people with years of panic attacks. That’s an observation from our clinic, not a trial result. We tell patients the same thing.

We prefer consecutive or near-consecutive days. One session every other weekend gives you a lovely evening and very little lasting change. The Toyama sleep study used seven sessions in two weeks, which matches how we usually schedule a first course.
A realistic timeline
Session one often feels strange. Session two is when most people let go. By the end of the first week, the common report is better sleep and fewer 3 a.m. wake-ups.
The tight chest before meetings and the constant background worry tend to soften later. And they stay softer when life at home changes too. We won’t promise a date. Some people feel a difference after two sessions. Others need the full two weeks plus home changes before anything moves.
What Shirodhara won’t do on its own
It won’t fix a job that keeps you online until midnight. It won’t replace sleep. If you finish a course and go straight back to 1 a.m. screens and skipped breakfasts, the calm fades within a few weeks. Who Is Shirodhara for Anxiety Best Suited For?
People who tend to do well
- Stress-driven anxiety with a clear trigger, such as work pressure, exams, caring for a sick parent or a big life change.
- Anxiety that shows up mostly at night, as racing thoughts, trouble falling asleep or waking at 3 a.m.
- IT professionals and shift workers with screen fatigue, tight necks and irregular hours. We see a lot of them at the Gachibowli branch.
- Anyone who wants a drug-free support alongside the therapy or medication their doctor has already prescribed.
When we postpone or say no
We’ll usually hold off on Shirodhara if you have any of these on the day:
- Fever, a cold, sinus congestion or a heavy, blocked head.
- A recent head, neck or spine injury, or recent surgery in that area.
- Wounds, infection or an active rash on the scalp or forehead.
- Very low blood pressure or a tendency to faint.
- Pregnancy, unless the physician has assessed you in person and agrees.
- A heavy meal in the last hour or two.
If panic attacks feel unmanageable, if you’re having thoughts of harming yourself, or if anxiety is keeping you from work or from leaving the house, please see a psychiatrist or clinical psychologist first. Shirodhara can support that care later. It can’t replace it.

Can You Have Shirodhara While Taking Anxiety Medication?
In most cases, yes. Shirodhara is an external therapy. Nothing is swallowed, so drug interactions with SSRIs, benzodiazepines or sleep tablets aren’t the concern they would be with internal herbs. The physician still needs your full medication list, because internal Ayurvedic medicines are sometimes prescribed alongside a course, and those do need checking.
We ask two things of every patient on medication. Tell your psychiatrist or GP that you’re starting Shirodhara. And change doses only with the doctor who prescribed them. Shirodhara with therapy and daily habits
It sits well next to CBT or counselling. Patients often say the calm after a session makes it easier to use the techniques their therapist teaches. At home, the habits that help most are simple. Fixed sleep and wake times. A warm dinner before 8 p.m. Less caffeine after lunch. Some patients add a short self-oil massage of the scalp and soles before bed, based on the Abhyangam principles our therapists show them.

Why not just try it at home?
Home Shirodhara kits are easy to buy online. The trouble is control. Oil that’s too hot can scald, oil that cools mid-session defeats the purpose, and a slippery floor after 2 litres of oil is a real hazard. The stream height and rhythm matter more than people expect.
